Don’t miss important health communications!
Please update your home address with your GP practice so you continue to receive important health communications, including:
- Screening invites
- Appointment details
- Results letters
If your contact details are not up to date, you may miss important information about your health. This is particularly important if you are moving house, graduating or leaving the country.
What you need to do
You can notify your GP practice of a change of details by:
- Completing a Contact Details Update Form on your GP practice website
- Contacting your GP practice directly
- You can also contact your GP practice by email; check their website for the email address.
Please include:
- Your name (and the names of anyone in your household affected by the move)
- Your current address
- Your date of birth
- Your new address
- The date the change takes effect
- If you are moving out of Scotland, please notify your GP practice.
Note: Updating your details with university or college administration will not update your records at your GP practice!
